Key Personnel Bios

Edward J. Jensen, P.E., C.S.P. -

President

Former Air Force Captain Edward Jensen has investigated safety-related accidents and ensured safety compliance since 1969. From growing up and working on the family farm to managing contracts supplying the world’s oil fields, he has experience across many engineering branches. The topics include: accident reconstruction; codes, contracts, and claims; electrical failures and fires; fires and explosions; structural and mechanical failures; workplace safety

Sergio Garibay, P.E., ACTAR -

Associate

Sergio Garibay, a former construction coordinator and mechanical equipment designer, has practiced engineering and investigated failures/accidents across the spectrum of agricultural, mechanical and civil installations. He has working experience in the application of codes and the incorporation of contracts under these circumstances. These topics include: repair of heavy machinery and equipment; accident reconstruction; codes, contracts, and claims; fires and explosions; structural and mechanical failures.

William J. Arnoult, Ph.D., P.E. -

Associate

Dr. Arnoult is a forensic materials engineer who specializes in the analysis of materials and metallurgical failures that pertain to matters in litigation. His professional experience in failure analysis, fracture analysis, and metallurgy is since 1969, and his professional experience in forensic engineering is since 1975. Topic include: structural and mechanical failures; fires and explosions; electrical failures and fires.

Elizabeth Jensen, Ph.D., P.E. -

Associate

Dr. Elizabeth Jensen, a fire protection engineer, former volunteer firefighter, and EMT, has analyzed spectroscopic images/data from radio to infrared since 1996 and is an expert on studying magnetic fields in plasma using remote sensing. She has hands-on practical experience in applied physics from configuring experimental radio frequency telescopes to measuring electrical and acoustic energy from fracturing core samples. Topics include: accident reconstruction; electrical failures and fires; fire protection engineering; fires and explosions; structural and mechanical failures; workplace safety.

How reporting close calls can prevent future incidents

Posted on: January 12th, 2012

“Near misses and resulting inspections may help prevent an injury or even a fatality, but an investigation cannot take place if the near miss is not reported accurately. Therefore, setting up a successful safety management program to ensure near misses are reported and investigated is an important step in reducing occurrences of serious incidents.” “OSHA [...]
